I greatly doubt in most cases that it's user error. I'm sure in some where people are to lazy to try and get that rear bolt tight they leave it loose and the vibration and the mount slamming down will cause it.
Most of the time its due to the fact that your driving an Econo-box that you've upgraded to make more hp/tq and its not built to take it. Sure, the car will be fine for a while, but eventually things are going to break.
Its a fairly simple thing to avoid. Make sure your mounts are tight, and it'll last for quite some time. Add the 4th mount, and it'll last even longer. It's by no means hard to run a 4th mount, even with the stock cross member, if your able enough to fab it up. A lot of people aren't, most of those people tend to have money so, go out and buy a traction bar lol.
I'm also planning on switching to Innovative mounts, or atleast the innovated rear mount. The Hasport mounts only use 2 bolts to hold it to the cross member, where the Innovative use 3. Using more surface area will help support. I don't know if the aluminum Innovatives use three, but I know the steel set does.
