Default I'm completely losing power at 4000 RPM???

This may be a very easy question, but this has never happened to me so I'm not sure what it is. This has just started a few days ago, whenever I get to about 4000 rpm's all the sudden i completely lose power to the wheels, the engine is still revving but the entire car just slows down. This only happens if the throttle is near wide open, if I slowly drive it up to 4000 it won't happen. But if I am running it wide open, and a hit around 4000 rpm's you can feel something vibrating badly, as well as a sort of high pitched whistling noise. I can't really describe it any better than that, because I only let this happen for about a split second. The car has 43,000 miles on it. I'm not sure if this has anything to do with it, but lately when I shift into 2nd it makes a loud clanking noise as well as it is much harder to get into gear than the others. Could the loss of power be a vacuum leak? Does this sound right? This is really starting to piss me off, so any help you guys have would be great. I guess I'll just take it to the dealer as soon as possible. Thanks.
