Lol I've been to louisville a few times to visit my brother in law. I thought it was a pretty nice place... but then again my brother in law is an Anesthesiologist. I guess his side of town is the nice side.
Seriously though, I would go to the emergency room. Some hospitals are not for profit hospitals. They can't turn anyone away, regardless of whether or not they can pay. My friend did something similar (well, actually in a bar fight lol), and injured his ankle. It was swolen like yours. His turned out to be the lateral malleolus of his fibula (bottom of the smaller leg bone). The first hospital told him it was his ankle and gave him a walking cast. After a week of excrutiating pain, he went to another hospital and they realized it was the fibula, not the ankle, and ended up putting in titanium mesh and screws.
