Good luck getting your plate anytime soon. I've been waiting for close to 2 months now for a custom plate for my Miata. It took close to two and a half months for the two other plates.

Yep, license and registration (or bill of sale if unregistered). $25 for the prestige (custom) plate and $20 for tag fee.
