Make sure all your connections at the ecu are good, I had the same issue in the past turned out when you de-pin wires they loosen up and don't make a very strong connection.check the connector that has crank angle sensor

Well the ecu should flash, be sure to check anyways..

The common problem is people get the Map, and tps plugs mixed up..

You need to go threw and test each to be sure the connection are right..

You need to be sure that the wiring is right before you just throw parts at it..

    So...

code 15 is ignition output signal/ignition control module in the dizzy

code 16 is saying an injector is bad, or not firing right..

My advice is to check the wires from the ECU to both the dizzy, and to the injectors.. It's a total of like 10 wires!
Get a wiring diagram, and check for continuity on each.. If your checks are good, then check the parts!!

You have to ohm the injectors, and also check ignitor in the dizzy.

I say this because you said the car had a wire tuck at some point..

bad ground. check the ground strap from engine to chassis, and from trans to chassis (DA integras with a B should have one). clean the grounds, and hook up heavy gauge ground wires if they are missing...

i've seen many have the issue you describe and it be a ground. sounds like it is hitting a rev limiter about 3.5k.
You're right - I have no idea what I'm talking about...
