i've had a manifold that was warped out of the box. this could cause the coolant to seap over to the #4 runner. if you didn't mix the lines this could be your problem. you have to take it back off and have a machine shop surface it. throw it back on with a new intake gasket and you should be good to go.
This could def be a possibility too. Good advice.
