    Put in 5th and brake will help to keep the crank still so you can break loose the bolt. You don't believe me? I've done this many times already. I have Unorthodox Pulley so that holder won't do me any good. That method is the only one for me. Try it, it doesn't hurt anything. Most important, you need a long steel pipe to use it as a long breaker bar.
