what all parts do i need to do it?

you would need the front knuckles, rotors, pads, and calipers and brackets
rear you would need the trailing arms and knuckles and you don't need to have the lower control arms because you could use regular integra LCA's , rotors, calipers, caliper bracket, and pads

what are you going to put this on?

Here is a JDM DC '98 Spec Integra Type R 5 Lug Front and Rear Disc Brake Conversion! Larger rotors and calipers for improved braking! Comes in a 5 x 114 bolt pattern. Includes all brake components, front knuckles, and rear trailer arms. This set requires the use of 36mm axles from a JDM '98 spec ITR or you can modify your existing axles to 36mm splines. (USDM B-series axles are 32mm). There are no brake lines, e-brake cable, shocks, springs or lower control arms included in this set.

That should answer your axle question too.
