You have to be 18+ to get a tattoo. You also have to have to have some sort of id (Driver's License, Birth Certificate, etc) You CANNOT sign for someone underage. They stopped that because people were getting infants tattooed.

Tell your ex to be like everyone else, wait till shes legal (we all had to do it) and get something personal to her (not some piece of flash off the wall). It will be worth it.

Just like others' advice, no reputable tattoo parlor is going to tattoo a minor. You get in major trouble for that....
