Blank Traffic ticket form????

Hey can any of you guys find me any kind of blank traffic citation that i can doctor up for a joke on one of my friends . Ive looked all over google and dont really come up with much?

Just get him arrested. That'll show him. Or duct tape a bag of baking powder under his car and call the ATF and say there is a car transporting a bunch of coke..... Do it from a payphone with no cameras though....

i know they make fake parking tickets, but not sure about other things. if you just google 'fake parking ticket' or 'prank ticket' or something to that effect, theres tons of results.
